Home Health Speech Therapist - Contingent

Key Responsibilities

  • Assesses, plans, and delivers speech-language pathology services in home health settings
  • Develops and revises care plans in collaboration with physicians and clients
  • Educates clients and caregivers on treatment and recovery techniques
  • Documents clinical findings, progress, and updates in accordance with agency policy
  • Participates in quality improvement and interdisciplinary coordination
  • Supervises HHAs and mentors new staff as assigned

Minimum Qualifications

  • Mets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association
  • Meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certificate
  • Registration or license to practice as a speech pathologist in the state of practice if applicable
  • Maintains clinical practice skills, learning, and adherence to agency’s policies and procedures on an ongoing basis
  • Functions in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families
  • Has current Driver’s license and reliable transportation to and from work site
  • Accurately documents observations, evaluations, and interventions pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system
  • Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail)
  • Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan of care and plans ahead for PTO or other time off
  • Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program

Mental and Physical Requirements

  • Lifts, stands, and walks 10% of time
  • Bends, stoops, and twists 10% of time
  • Positions and lifts patients 10% of time
  • Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time)
  • Must be able to adapt to frequently changing work parameters
  • Must be able to travel to various TH/THHS worksites or various work locations as required
