Role: Speech Language Pathologist providing home-based evaluation and treatment for adult and geriatric clients. Responsibilities Assess speech, language, cognitive-communication, and swallowing disorders.Develop and implement individualized care plans.Deliver evidence-based interventions in clients’ homes and community settings.Collaborate with caregivers, nurses, and other clinicians to promote safety, independence, and quality of life.Document progress and outcomes following regulatory and agency standards.
