Home Health Salaried Physical Therapist Full Time

About the role

Provide in-home physical therapy services to help patients recover from illness or injury, focusing on assessments, treatment planning, and discharge planning.

Responsibilities

• Conduct initial and ongoing evaluations of patients.
• Administer tests to determine patient goals and develop detailed care plans.
• Deliver prescribed physical therapy treatments.
• Maintain accurate and timely documentation of patient care.
• Collaborate with healthcare teams to coordinate patient care.
• Supervise personnel such as Physical Therapist Assistants (PTAs) or Home Health Aides (HHAs) where applicable.
• Complete OASIS assessments in compliance with regulatory requirements.
• Ensure compliance with company policies, state regulations, and licensure requirements.
• Communicate effectively with staff, patients, and families, serving as a clinical resource.

Requirements

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification, Driver’s License, and Proof of Automobile Insurance.

Preferred Experience

• Home health therapy experience.
• Completion of OASIS documentation.
