About the Role
Full-Time Home Health Physical Therapist role focused on providing patient-centered care in patients' homes.
Responsibilities
- Perform initial and ongoing assessments to evaluate mobility, strength, and functional status.
- Administer, evaluate, and interpret tests to set and monitor treatment goals.
- Develop and document individualized plans of care, including goals, progress, and outcomes.
- Deliver physical therapy services aligned with physician orders, ensuring quality care.
- Maintain comprehensive documentation to support billing and compliance, with minimal supervision.
- Coordinate with healthcare team, patients, and caregivers, providing education on home exercise plans.
- Supervise other personnel (PTA, HHA) as applicable and complete required assessments like OASIS within scope.
- Ensure compliance with policies, regulations, and licensure standards.
Qualifications And Skills
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR, driver's license, and proof of automobile insurance.
- Preferred: Experience in home health therapy and OASIS documentation.
