Registered Nurse Preceptor

• Mentors, orientates, educates and trains assigned professional and unlicensed client care personnel in helping them to transition into their roles.
• Evaluates clinical competency of employees that are assigned by Clinical Supervisor or designee to ensure quality standards of care are met.
• Oversees home health services for clients within the community and performs comprehensive client care, and case management as assigned.
• Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans taking into consideration quality measures and financial aspects of treatment.
• Buils and maintains respectful relationships with others.

Schedule

This is a full time position, Monday - Friday with an on-call rotation.

Coverage Area

This position reports to the Mendota Heights, MN office and provides services in Mendota Heights and surrounding cities.

Responsibilities

• Reflects the company’s mission and values in developing and maintaining professional working relationship with others.
• Provides direct instruction and guidance to clinical staff on clinical procedures, protocols and best practices, in all client care areas including, and not limited to:
• Initial and periodic assessment of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes.
• Collaborates with physicians/non-physician practitioners and the home health team to develop and update the client’s home health plan of care at least every 60 days or more often as changes occur.
• Collaborates with physicians/non-physician practitioners and the home health team to develop and update the client’s home health plan of care at least every 60 days or more often as changes occur.
