Home Health RN - Part Time PRN Weekends

Role Overview: This position involves providing home health nursing care on a part-time, PRN basis, primarily supporting patients in their homes to promote recovery, safety, and independence. Key Responsibilities Serve as the primary clinical partner for patients and families, delivering personalized, one-on-one care.Conduct comprehensive in-home assessments to develop patient-centered care plans.Administer skilled interventions such as wound care, infusions, medication management, and post-operative care.Educate and empower patients and caregivers to manage symptoms and treatments confidently.Monitor patient progress, identify early changes, and communicate with care teams to adjust plans and prevent hospitalizations.Coordinate care across providers, ensuring continuity and quality outcomes.Complete accurate documentation supporting compliance and quality metrics. Qualifications & Skills Active RN license in the practice state with at least one year of clinical experience.Patient-focused, organized, and motivated to deliver high-quality care.Valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
