Home Health Registered Nurse - Part Time PRN Weekends

The Registered Nurse provides skilled nursing care in patients' homes, following care plans and physicians’ orders while maintaining confidentiality and professional boundaries. Key Responsibilities Include Developing personalized care plans based on assessments, physician input, and client needs, including setting measurable goals.Educating clients and families to promote understanding and health management.Training primary caregivers to ensure optimal client functioning.Coordinating ongoing care through documentation and communication with physicians and caregivers.Delivering various home-based skills such as patient assessment, wound care, infusions, catheter care, and dressing changes. Qualifications And Skills Active RN license in the applicable state.Valid CPR/BLS certification and driver’s license.Preferred: Medicare Skilled Nursing experience, basic Oasis knowledge, and at least one year of RN experience.
