About the Role

Lead the delivery of exceptional patient care while shaping the success of a high-performing clinical team. As a Home Health Clinical Manager, you'll oversee the coordination and quality of care for patients throughout their home health journey, ensuring they receive the right care at the right time while supporting positive outcomes and an exceptional patient experience. This role is ideal for an experienced clinician who is passionate about developing others and driving excellence in patient care.

You'll provide leadership, mentorship, and clinical guidance to a multidisciplinary team of nurses, therapists, social workers, and home health aides while fostering a culture of collaboration, accountability, and continuous improvement. Working closely with physicians, referral partners, and community providers, you'll help ensure seamless care coordination, regulatory compliance, and operational success. Your expertise and leadership will directly influence patient outcomes, team performance, and the continued growth and success of the organization.

Location

Office Location: Bingham Farms, MI

Schedule

Monday to Friday, 8 a.m. to 5 p.m. Includes on-call administrative phone support rotation for after-hours field staff.

Requirements

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing OR Doctorate School of Physical Therapy
  • Current State License (RN or PT)
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage
  • Availability during all operating hours with flexibility for schedule disruptions

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) for RN candidates
  • Minimum 2 years of home health experience
  • Previous supervisory or management experience

Skills

  • Excellent observation, verbal, and written communication skills
  • Strong clinical judgment and ability to provide expert guidance on complex cases
  • Knowledge of business management and healthcare operations
  • Understanding of governmental regulations, Medicare/Medicaid requirements, and CHAP standards
  • Proficiency with home health EMR systems
  • Ability to assess and respond to varied patient, family, and staff needs
  • Strong leadership and personnel management capabilities
  • Ability to handle multiple priorities, deadlines, and frequent disruptions
  • Cultural sensitivity and tolerance for diverse lifestyles
  • Conflict resolution and problem-solving skills

Pay

Pay Range $93,500—$115,000 USD
