Home Health RN Case Manager (Office Position)

About the role

Safe Life Home Healthcare, LLC, a 4-star patient satisfaction organization founded in 2010, seeks a dedicated and experienced Nurse Case Manager for an on-site position in Lombard, IL.

Responsibilities

• Comprehensive patient assessment covering physical, psychological, and social aspects.
• Document patient observations and conditions during each visit.
• Curate a personalized care plan for each patient and ensure adherence.
• Educate patients or caregivers on essential self-care skills for home management of chronic conditions.
• Oversee medication protocols and management.
• Specialized care including wound and IV management, PICC Line, G-Tube, and Foley care.
• Collaborate with a diverse team, from physicians to therapists, ensuring an integrative care approach.

Qualifications

• Possession of, at a minimum, an Associate's Degree in Nursing or a Bachelor of Science degree in Nursing.
• Home Health experience and familiarity with its documentation are HIGHLY DESIRED.
• Capability to work autonomously or synergistically with a medical or therapeutic team.
• Sound understanding of Home Health care regulations and best practices.
• Versatility and empathy to operate in diverse living conditions.
