About the role

This is a field-based home health nursing role providing direct patient care in patients’ homes. Manage your own patient caseload while completing skilled nursing visits, assessments, patient education, care coordination, and documentation in the home setting. Work independently in the field while receiving strong support from experienced clinical leadership, including Clinical Manager oversight and LPN care support. Collaborate closely with LPNs and an interdisciplinary team to ensure high-quality patient outcomes and coordinated care throughout the episode.

Responsibilities

Integrates health care for clients from pre-admission to post discharge through the coordination and sequencing of the client’s care. Enhances patient/resident flow and interdisciplinary communication to promote early intervention. Evaluates outcomes based on integration among established clinical, financial, and utilization data. Functions in structured and unstructured health care settings, utilizing independent nursing judgment when integrating health care.

  • Has knowledge of and utilizes appropriate age-specific structured care methodologies, such as protocols, integrated clinical pathways, guidelines, and standards of care relating to the overall health care needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents.
  • Able to work with growth and development needs of client populations in clinical area. May work with clients in all age groups.
  • Coordinates, facilitates, and negotiates with patients/residents, family members, and others on the health care team.
  • Monitors, evaluates, and trends patient/resident responses utilizing structured care methodologies.
  • Fiscally responsible. Controls variances and duplication.
  • Demonstrates independence, assertiveness, and critical thinking when working with patients/residents and co-workers.
  • Documents clearly and solves problems.
  • Self-motivated and works with little direction.
  • Incorporates teaching/learning principles and adapts teaching to age group.
  • Facilitates groups and implements projects when necessary.
  • Establishes priorities, has strong flexibility, and organizational skills.
  • Knowledgeable about reimbursement for services provided.

Requirements

  • Bachelor's degree in Nursing strongly preferred.
  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department-specific competencies and certifications.
  • Depending on location, a valid driver's license may be required.

Schedule

  • Shift: 8-hour day shifts
  • Full-time, 40 hours per week
  • Primarily day hours, scheduled weekends, and occasional evenings

Pay

  • Salary range: $36.50 - $51.00 per hour
  • $10,000 sign-on bonus
