RN - Case Manager - Home Health

About the role

Provides professional nursing care to clients in a home health setting, supporting physicians and adhering to department policies.

Responsibilities

• Ensuring safe, quality care for clients of all ages through proper documentation, communication, and resource management.
• Conducting client assessments, developing and updating Plans of Care in collaboration with healthcare team members.
• Implementing nursing interventions, monitoring client progress, and communicating changes to physicians.
• Supervising and directing home health aides, coordinating care, and ensuring compliance with policies and regulations.
• Performing bedside lab tests, obtaining specimens, and maintaining accurate, timely clinical documentation.
• Prioritizing and organizing visits, demonstrating flexibility, and responding to on-call needs.
• Participating in educational activities for professional growth and staying current with healthcare developments.

Qualifications and Skills

Valid IL RN license, BLS certification, and one year of nursing experience. Effective communication, documentation, and clinical skills tailored to diverse client needs. Ability to work independently, organize schedules, and respond to emergency on-call situations. Knowledge of regulatory standards, OASIS data sets, and payor requirements.
