RN - Case Manager - Home Health

We are seeking a skilled Home Care RN/Field Case Manager to provide independent nursing visits and coordinate patient care across multiple counties. Responsibilities Include Assessing, planning, and implementing care for diverse patient needs, including wound care, PICC lines, TPN, and tracheostomy management.Coordinating with an interdisciplinary team to ensure seamless care delivery and communication.Performing documentation such as OASIS assessments, visit notes, and physician orders accurately and timely.Supervising home health aides and directing care activities in patients' homes.Monitoring and managing complex medical devices and administering infusions.Acting as a primary liaison with physicians, caregivers, and families to support patient wellness and independence. Qualifications And Skills Current IL RN license, BLS certification, and a valid driver’s license with proof of auto insurance.Minimum of 1 year nursing experience.Knowledge of Medicare home care benefits, regulatory standards, and documentation protocols.Strong case management skills, clinical competence in wound care, central lines, and medication infusions.Excellent communication, organizational, and supervisory abilities. Key Duties Include Providing skilled nursing assessments and education in patients' homes.Ensuring compliance with regulatory and documentation standards.Collaborating with healthcare team members for comprehensive patient care.Managing complex clinical procedures and supporting patient safety and independence.
