Home Health RN Case Manager

Responsibilities

• Play a hands-on role in developing and adjusting the plan of care for your patients, working alongside LPNs, therapists, and physicians
• Complete visits, OASIS assessments, and documentation with an eye toward the full patient picture, not just the task in front of you
• Carefully coordinate with the care team so nothing falls through the cracks
• Educate patients and families on diagnosis, medication, and home safety
• Advocate for your patients and help drive real outcomes

Requirements

• Active RN license in Oklahoma
• Home health experience is ideal, but we're also looking for experienced nurses from any clinical background
• Ready for a change and bring strong assessment and critical thinking skills
• CPR certification

Qualifications

• Experience in a clinical setting
• Strong communication and interpersonal skills
• Ability to manage multiple tasks and prioritize effectively

Schedule

• Primarily Monday through Friday
• The flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

Pay

• We offer flexible pay structures for this role, either salary or per-visit, whichever fits you best:
• Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
• Salary option available for the right candidate
