Home Health RN Case Manager
About the role
If you're a nurse who's ready to bring real clinical judgment back into your day instead of just checking off visits, this Home Health RN Case Manager role in Oklahoma City is worth a look. You'll work as part of a full care team, including LPNs and support staff, while playing a meaningful role in shaping the plan of care for your patients. It's the kind of nursing where you actually get to know your patients, not just their chart.
Responsibilities
- Play a hands-on role in developing and adjusting the plan of care for your patients, working alongside LPNs, therapists, and physicians
- Complete visits, OASIS assessments, and documentation with an eye toward the full patient picture, not just the task in front of you
- Cookordinate with the care team so nothing falls through the cracks
- Educate patients and families on diagnosis, medication, and home safety
- Advocate for your patients and help drive real outcomes
Requirements
We're Interim HealthCare of Oklahoma City, a locally owned, Veteran and nurse owned home health, hospice, and personal care agency that's been part of this community since 1999. We hold a 4.5 star CMS rating, and our team stays rooted in OKC through things like Meals on Wheels and our monthly Veteran-to-Veteran coffees, so when you join us, you're joining a company that shows up for this city, not just a name on a badge.
