We hold a 4.5-star CMS rating and are actively involved in local initiatives like Meals on Wheels and monthly Veteran-to-Veteran coffees.
Schedule
Full time, Monday through Friday, with flexibility to structure your day around your patients.
Pay
Flexible pay structures, either salary or per-visit, whichever fits you best:
- Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
- Salary option available for the right candidate

Responsibilities
- Play a hands-on role in developing and adjusting the plan of care for your patients, working alongside LPNs, therapists, and physicians
- Complete visits, OASIS assessments, and documentation with an eye toward the full patient picture, not just the task in front of you
- Coordinate with the care team so nothing falls through the cracks
- Educate patients and families on diagnosis, medication, and home safety
- Advocate for your patients and help drive real outcomes
Requirements
- Active RN license in Oklahoma
- Home health experience is ideal, but we also consider experienced nurses from any clinical background who are ready for a change and bring strong assessment and critical thinking skills
- CPR certification
- A desire for autonomy, real patient relationships, and a genuine say in the plan of care
