About the role
This is NOT a remote position.
Responsibilities
- Makes rounds to monitor patient’s overall health
- Develops an individualized plan of care in partnership with a primary care physician
- Works independently to deliver quality healthcare to patients
- Provides customized care and patient education about disease management
- Delivers specialized services such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management
Requirements
- Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
- Active CA Registered Nursing License
- AAS, BSN, MSN degree
- Reliable Transportation
- Current CPR or BLS Card

- Comfortable working in a home care setting

Pay
- Based on full-time schedule
Schedule
- Per Diem (12 RVE p/week)
- Must work 3 days per week: Mon, Wed, Thur or Tues, Thur, Sat or Sun, Tues, Fri
- Non-traditional schedules are available, i.e., Thursday-Monday
Location
- Area #45 covers the following cities: Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Village
Paid Training
Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite)
