Pay

• Visit rates: $50–$150 per visit, plus mileage and travel compensation
• Full-time RNs average $75,000–$85,000 annually

Schedule

• Full-time or part-time positions available
• Part-time minimum expectation: 5 visits or 10 hours per week

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs, and aides providing patient care
• Examine patients, perform assessments, document progress, and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet, and exercise
• Coach family members on the patient’s plan of care, medication, and home safety
• Ensure goals are met and coordinate discharge from services

Requirements

• Active Registered Nurse (RN) license in Colorado
• Graduate of an accredited school of nursing
• At least 1,200 hours of experience as an RN
• Healthcare CPR/BLS certification (demonstration course required)
• Home health care experience preferred
• Pass a background check and drug test (THC testing required)
• No COVID-19 vaccination required
