Home Health Registered Nurse - Full Time

Role Overview

This position is for a Registered Nurse (Home Health) focused on providing personalized, one-on-one nursing care in patients' homes. The nurse acts as the primary clinical partner, conducting assessments, delivering skilled interventions such as wound care, infusions, medication management, and post-operative care, and developing patient-centered care plans.

Key Responsibilities

• Serve as the main clinical contact for patients and families, promoting recovery, safety, and independence.
• Perform comprehensive in-home assessments to identify physical, emotional, and environmental needs.
• Provide tailored nursing interventions and educate patients and caregivers for effective symptom and medication management.
• Monitor patient progress, communicate with healthcare teams, and adjust care plans to prevent hospitalizations.
• Ensure accurate documentation supporting quality and compliance.
• Coordinate care across providers to ensure continuity and high-quality outcomes.

Qualifications & Skills

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
