Home Health Registered Nurse - Full Time

The Registered Nurse provides skilled nursing care in patients' homes, following care plans and physicians’ orders while maintaining confidentiality and professionalism. Key Responsibilities Include Developing care plans based on assessments, physician input, and patient needs with measurable goals.Educating patients and families tailored to individual needs.Training primary caregivers for optimal patient support.Coordinating care through documentation and communication with physicians and caregivers.Delivering care using skills such as patient assessment, wound management, infusions, catheter care, and dressing changes. Qualifications And Skills Active RN license in the applicable state.Valid CPR or BLS certification.Valid driver’s license. Preferred Qualifications Include Experience with Medicare Skilled Nursing.Basic understanding of Oasis documentation.At least one year of RN experience in healthcare settings.
