About the role
We are seeking a dedicated Registered Nurse (RN) for a full-time home health role, providing personalized nursing care in patients' homes.
Key Responsibilities
- Serving as the primary clinical partner for patients and families, promoting recovery, safety, and independence.
- Conducting comprehensive in-home assessments to develop patient-centered care plans.
- Performing skilled nursing interventions such as wound care, infusions, medication management, and post-operative care.
- Educating and empowering patients and caregivers for effective symptom, medication, and daily care management.
- Monitoring patient progress, identifying early changes, and communicating with the interdisciplinary team to adjust care plans.
- Coordinating care across providers to ensure continuity and high-quality outcomes.
- Completing accurate documentation to support quality and compliance.
Qualifications and skills
- Active RN license in the practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Reliable transportation, a valid driver's license, and CPR certification or ability to obtain it.
