Position Overview The Registered Nurse – Home Health Full Time provides and documents skilled nursing care in patients' homes following care plans and physician orders, maintaining confidentiality and professional boundaries. Key Responsibilities Develop care plans based on assessments, physician input, and patient needs, setting measurable goals.Educate patients and families tailored to their specific conditions.Orient and train primary caregivers to optimize patient care.Coordinate ongoing care through documentation and communication with physicians and caregivers.Deliver care using skills such as patient assessment, wound care, infusions, catheter care, and PICC line dressing changes. Qualifications & Skills Active RN license in the applicable state.Valid CPR certification and driver's license.Preferred: Medicare skilled nursing experience, basic Oasis understanding, and 1-year RN experience in healthcare.
