Home Health Registered Nurse - Full Time

Role Overview: A full-time Registered Nurse (RN) provides comprehensive home health care in the Twin Cities Metro and suburbs, supporting Start of Cares and case management activities. Key Responsibilities Serve as the primary clinical partner for patients and families, delivering personalized, one-on-one nursing care at home.Conduct in-home assessments to evaluate physical, emotional, and environmental needs, developing patient-centered care plans.Perform skilled interventions such as wound care, infusions, catheter care, medication management, and chronic disease management.Educate and empower patients and caregivers to effectively manage symptoms, medications, and daily activities.Monitor patient progress, identify early changes in condition, and communicate with physicians and care teams to adjust plans and prevent hospitalizations.Coordinate care across providers, ensuring continuity and high-quality outcomes.Complete accurate documentation supporting compliance and quality metrics. Qualifications & Skills Active RN license in the state of practice.Minimum of one year of clinical nursing experience.Patient-focused, organized, and motivated to deliver high-quality care.Valid driver’s license, reliable transportation, and auto insurance.CPR certification or ability to obtain it.
