Home Health PTA - Full Time

Position Overview Seeking a Physical Therapy Assistant (PTA) to deliver therapy interventions and treatments under the direction of a physical therapist, following the physician’s plan of care. Responsibilities include assessing patients, providing skilled therapy, educating patients and caregivers on device use and rehabilitation techniques, and collaborating with interdisciplinary team members to optimize patient outcomes. The role involves communicating updates, reporting patient conditions, planning discharge, and ensuring documentation accuracy. Adherence to ethical standards and confidentiality is essential. Qualifications & Skills Graduation from an accredited PTA programCurrent, unrestricted state license as a PTAValid CPR certificationStrong organization, communication, and time management skillsExperience in home health therapy and OASIS documentation preferred Key Duties Provide direct physical therapy per therapist’s instructions and physician’s planUpdate patients and caregivers about the care planReport patient observations and progressAssist with device education and rehabilitation strategiesCoordinate discharge planning and instructionsMaintain accurate and timely documentationComply with professional standards and confidentiality requirements
