About the role
Provide personalized rehabilitation care in patients' homes as a Home Health Physical Therapist Assistant.
Responsibilities
- Implement treatment plans developed by Physical Therapists
- Perform manual exercises and therapeutic modalities
- Document patient progress
- Collaborate in care coordination and discharge planning
- Support other healthcare team members
Requirements
- Current unrestricted Physical Therapy Assistant license in North Carolina
- Valid driver’s license, auto insurance, and reliable transportation
- CPR certification
- TB screening due to patient-facing role
Qualifications
Preferred: Home health experience
Pay
- $41.00 - $57.00 per visit/unit
- $64,000 - $87,500 annually
