About the role

Provide home-based physical therapy to patients in the Raleigh area, focusing on assessments, treatment planning, and delivering therapeutic services to promote recovery from illness or injury.

Responsibilities

• Conduct patient evaluations and develop individualized plans of care.
• Update treatment plans based on patient progress and evolving needs.
• Document patient progress and maintain accurate records.
• Collaborate with healthcare teams, patients, and caregivers to ensure coordinated care.
• Supervise therapy personnel where applicable.
• Complete OASIS assessments as permitted.

Requirements

• Degree in Physical Therapy from an accredited program.
• Current unrestricted state licensure in North Carolina.
• Proficiency in patient assessments and clinical documentation.
