Home Health PT Physical Therapist PRN

About the role

A licensed Physical Therapist is needed to provide home health care services, enabling patients to recover in their own homes within the Raleigh area.

Responsibilities

• Performing initial and ongoing assessments of mobility, strength, and function.
• Administering, evaluating, and interpreting tests to set functional goals and outcomes.
• Developing and documenting personalized care plans with goals, progress, and expected results.
• Delivering therapy services per physician orders, maintaining documentation, and modifying treatment as needed.
• Planning discharges, educating patients and caregivers on home exercises, and coordinating with healthcare team members.
• Supervising other personnel (PTA, HHA, OTA) where applicable and completing assessments like OASIS within scope.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Proficiency in assessments, documentation, and compliance with policies.

• Effective time management and relationship-building abilities.
• Experience in home health therapy and OASIS documentation.
