Home Health Physical Therapist PT PRN

Position Overview Seeking a Physical Therapist (PT) to provide home health care services, enabling patients to recover in their own homes. The role involves performing initial and ongoing assessments of mobility, strength, and function, and administering tests to set and evaluate treatment goals. Responsibilities include developing and updating individualized care plans, delivering prescribed therapies, documenting patient progress, and conducting discharge planning with patient education. The therapist collaborates with healthcare team members, maintains accurate documentation to ensure compliance, and supervises personnel such as PT assistants and home health aides where permitted. Additional duties include completing OASIS assessments as allowed by regulation, ensuring adherence to policies, standards, and legal requirements, and facilitating effective communication among staff, patients, and families. Qualifications & Skills Degree in Physical Therapy from an accredited programCurrent, unrestricted state licenseValid CPR certificationPreferred: strong organization, communication, attention to detail, time management, relationship-building, and experience with home health therapy and OASIS documentation Key Duties Conduct assessments and interpret tests to determine patient needsCreate and modify individualized treatment plansDeliver therapy services in accordance with physician ordersDocument patient progress and modify care plans as neededEducate patients and caregivers on post-discharge exercisesCoordinate with healthcare team and supervise staff as permittedComplete required assessments and ensure compliance with regulations
