Responsibilities

• Deliver care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care
• Monitor patient conditions and report significant changes to the RN Case Manager and healthcare team
• Assist patients and caregivers with education and self-care techniques
• Implement patient safety plans and utilize available community resources
• Collaborate with the interdisciplinary care team to ensure exceptional patient outcomes
• Document clinical findings, nursing interventions, and medication updates accurately and timely
• Identify opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits
• Provide compassionate, professional support to patients, families, physicians, and colleagues

Requirements

• Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
• Current Massachusetts Licensed Practical Nurse (LPN) license
• Minimum of one (1) year of clinical nursing experience
• Valid driver's license, reliable transportation, and current auto insurance
• Ability and willingness to travel throughout the assigned territory

Preferred Qualifications

• Previous home health or hospice experience

Physical Requirements

• Sit, stand, bend, lift and move intermittently
• Able to lift 50-100 lbs.

Pay

Competitive compensation based on experience with a range between $32-$38/hour.
