Position Overview: The Licensed Practical Nurse (LPN) Part Time PRN provides skilled nursing care, documenting patient interactions according to care plans and physicians’ orders. Responsibilities include delivering quality care, communicating with healthcare providers, recognizing patient needs, and educating clients and families. Care is delivered in patients' homes using skills such as wound care, infusions, catheter care, and assessments. Requirements: Active LPN license in the state, valid CPR, and a driver’s license. Preferred Qualifications: Medicare skilled nursing experience, familiarity with Oasis, 1-year LPN experience in home health or healthcare, and Home Care Home Base experience. Key Skills: Clinical competence, communication, patient education, assessment, and documentation skills. Key Duties: Provide hands-on care, monitor patient conditions, implement interventions, and communicate with healthcare teams to ensure optimal patient outcomes.
