About the role

The Licensed Practical Nurse (LPN) works under the supervision of the Care Manager to provide comprehensive skilled nursing services in clients' homes. This position requires a strong understanding of Medicare, Medicaid, and insurance requirements, as well as familiarity with agency policies and procedures.

Responsibilities

• Affords assistance to physicians and registered nurses in the care of clients through the provision of comprehensive skilled nursing services aimed at maximizing rehabilitation and achieving established goals.
• Administers comprehensive skilled nursing services to assigned patients in the case load.
• Remains aware of Medicare, Medicaid, and commercial insurance requirements, and adheres to the scope of service.
• Develops and implements a plan of care, treatments, and procedures that contribute to the physiological and psychosocial outcomes of the patient.
• Collaborates with the primary RN in providing quality patient care as outlined in the Plan of Care.
• Serves as a supplement to the nursing care needs of the client as directed by the RN.
• Assists the RN or physician in performing specialized procedures and duties.
• Builds and maintains trusting relationships with a multidisciplinary team to support patient care needs.
• Participates actively in multi-disciplinary team meetings as assigned.
• Documents clinical outcomes of interventions accurately and consistently, including completing and locking patient notes and verbal orders within 24 hours.

Requirements

• Program Graduate, Licensed Practical Nurse (LPN)
• Nursing experience in an acute care setting or home care setting (preferred)
• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC)
• Valid Drivers License Required

• Education: Program Graduate, Licensed Practical Nurse
• Experience: Nursing experience in acute care setting or home care setting (preferred)

Pay

The hiring range is $28.50 - $39.00 hourly, actual pay determined by program, work commitment, and business needs.

Schedule

The LPN may be required to work every other weekend depending upon client census.
