Home Health Physical Therapists

About the role

We’re a growing Medicare-certified Home Health agency on O’ahu focused on fall prevention, mobility, and helping seniors stay safe and independent at home.

We're currently hiring licensed Physical Therapists and Assistants who are passionate about improving the lives through quality, in-home and community-based therapy.

Responsibilities

• Provide in-home physical therapy visits
• Complete evaluations, reassessments, and fall risk assessments
• Document accurately and communicate progress with the care team

Requirements

• Current Hawaii license as PT or PTA
• 1+ year clinical experience (home health preferred)
• Strong documentation and communication skills
• Driver’s license + reliable transportation

Pay

• Physical Therapist (PT): $80 per visit
• Physical Therapist Assistant (PTA): $55–$60 per visit

Location

O’ahu, HI (reliable transportation required)
