Home Health Occupational Therapists

About the role

Always Best Care is a growing Medicare-certified Home Health agency serving patients throughout O‘ahu. We are currently hiring licensed Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) who are passionate about improving quality of life through skilled, patient-centered home health therapy.

Responsibilities

• Provide skilled occupational therapy services in patients' homes
• Perform initial evaluations, reassessments, and fall risk assessments (OT)
• Develop and implement individualized treatment plans
• Improve independence with activities of daily living (ADLs), mobility, safety, and home management tasks
• Educate patients and caregivers on adaptive techniques and fall prevention strategies
• Collaborate with nurses, therapists, physicians, and caregivers
• Maintain accurate documentation in accordance with Medicare home health standards

Requirements

• Current Hawaii license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)
• Minimum one year of clinical experience (home health experience preferred)
• Strong communication, organizational, and documentation skills
• Ability to work independently in the field
• Valid driver's license and reliable transportation

Schedule

Flexible / Per Diem (PRN) - Build Your Own Caseload

Pay

• OT Pay: $80 per visit or $90,000-$110,000 annually (based on caseload)
• COTA Pay: $60 per visit

Location: O‘ahu, HI (Reliable transportation required)
