Home Health Physical Therapist (PT) - Per Visit Pay

About the role

Provide patient-centered care to geriatric patients in a home setting, conducting in-home evaluations and developing individualized plans of care. Focus on therapeutic interventions such as functional strengthening, gait training, balance, and fall risk reduction to enhance mobility and independence.

Responsibilities

• Managing your caseload and scheduling visits based on your availability and patient needs.
• Collaborating with a multidisciplinary team to ensure coordinated care and optimal patient outcomes.
• Utilizing an innovative app for managing logistics, documentation, and communication, streamlining workflow and administrative tasks.
• Ensuring compliant charting with integrated EMR system and maintaining all necessary certifications and licenses.

Qualifications and skills

• Valid California Physical Therapist license
• CPR or BLS certification
• Driver's license and auto insurance
• Physical and TB test (within the last year)
• Professional liability and business license (if applicable)
