Home Health Physical Therapist PT Full Time

Position Overview

Seeking a Physical Therapist (PT) to provide home health care, enabling patients to recover in their own homes.

Responsibilities

• Conducting initial and ongoing assessments of mobility, strength, and function
• Developing and implementing personalized treatment plans
• Delivering therapy services per physician orders
• Documenting progress and modifying goals
• Managing discharge planning with patient education on home exercises
• Collaborating with healthcare teams
• Supervising personnel (PTA, HHA)
• Completing assessments like OASIS where permitted

Qualifications & Skills

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR
• Driver’s license and proof of auto insurance
• PREFERRED: strong organization, communication, attention to detail, time management, relationship-building, and home health experience
