About the Role

The Physical Therapist (PT) provides home health physical therapy services to patients, facilitating recovery in their own homes. Responsibilities include conducting assessments, developing and modifying plans of care, delivering therapy per physician orders, and documenting patient progress to ensure optimal outcomes.

Responsibilities

• Perform assessments and interpret tests to set functional goals
• Develop, implement, and revise individualized plans of care
• Deliver therapy services and document progress effectively
• Coordinate with healthcare team members and educate patients/caregivers on home exercise programs
• Supervise PT aides or home health aides where applicable
• Complete required assessments (e.g., OASIS) and ensure compliance with policies
• Maintain effective communication with staff and families

Requirements

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR certification, driver’s license, and proof of auto insurance

Skills

• Attention to detail and time management
• Experience in home health therapy and OASIS documentation (preferred)
