Seeking an experienced Home Health Physical Therapist to provide patient-centered care for geriatric patients in a flexible, autonomous role within diverse neighborhoods, emphasizing urban, vibrant environments with opportunities for growth and independence.

Responsibilities

• Conducting in-home evaluations and developing individualized care plans.
• Implementing therapeutic interventions such as strengthening, gait training, balance, and fall risk reduction.
• Managing a personal caseload, scheduling visits based on patient needs and availability.
• Collaborating with a multidisciplinary team to ensure coordinated patient care.

Requirements

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license, auto insurance, and recent TB test.
• Professional liability and business license, if applicable.

Skills

• Spanish proficiency is a plus due to regional demographics but not mandatory.
