Responsibilities
- Accurately assess clients at admission and other required time points using appropriate standardized documentation
- Assess the client’s condition and needs, determining the agency’s ability to meet those needs in the home care setting
- For therapy-only cases, inform the client/caregiver about the agency, care/services available, client obligations for payment, and rights and responsibilities before initiating care
- Make appropriate referrals for evaluation/care to other disciplines and services, coordinating care to ensure effective and efficient delivery
- Report changes in client condition to the physician in a timely manner and obtain orders for changes in the plan of treatment
- Initiate and implement preventive, therapeutic, and rehabilitative procedures and techniques
- Perform manual therapeutic exercises, massage, and/or manual traction as established by the plan of care
Requirements
- Degree from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association, including an internship
- Licensed to practice as a physical therapist in the state of practice
- Minimum of one (1) year of experience as a physical therapist or completion of a preceptor program for 6-8 weeks
- Current working knowledge of conditions of participation and home healthcare experience preferred
- Able to function with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with clients and families
- Current driver’s license and reliable transportation to and from work sites
