Home Health Physical Therapist (PT)

About the Role

Lorian Health is seeking a qualified Physical Therapist (PT) to direct, supervise, evaluate, and provide physical therapy services to patients/clients in the home as prescribed by the attending physician. Responsibilities

• Provide treatment as ordered by the attending physician, dentist, or podiatrist.
• Assist in developing and updating the plan of treatment or Plan of Care (POC).
• Assist the physician in evaluating the patient’s level of function.
• Observe, record, and report information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advise, consult, and instruct family and other Lorian personnel in the patient’s therapy program.
• Teach and supervise other healthcare personnel when appropriate.
• Evaluate the home environment and make appropriate recommendations.
• Participate in in-service education programs.

Qualifications

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Current CPR certification.
• Licensed driver with an insured automobile that meets Lorian Health requirements.
• Ability to verbally communicate in the language of the client and document all client-related activity.

Pay

This position is based on a pay-per-visit compensation model, ranging from $110 to $170 per visit, commensurate with experience and depending on the type of visit.
