What You'll Do

  • Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
  • Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
  • Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
  • Collaborate for Optimal Outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.

What You'll Need

  • Valid California Physical Therapist license
  • CPR or BLS certification
  • Driver's license and auto insurance
  • Physical + TB test (within the last year)
  • Professional liability and business license (if applicable)
