About the Role

As a Home Health Physical Therapist, you’ll provide skilled, one-on-one therapy services in the comfort of patients’ homes, helping them regain strength, mobility, and independence. You’ll assess patient needs, develop individualized treatment plans, deliver evidence-based interventions, and educate patients and caregivers to support safe recovery at home. This role offers the opportunity to practice at the top of your license with greater flexibility, autonomy, and meaningful one-on-one patient relationships, whether you’re experienced in home health or transitioning from hospital, outpatient, acute care, or skilled nursing settings.

Responsibilities

• Provide skilled, one-on-one physical therapy services in patients’ homes.

Requirements

• Current licensure as a Physical Therapist in the State of Michigan.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care experience preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.
