Responsibilities

• Deliver individualized physical therapy services (evaluations and treatments) in the patient's home.
• Perform therapeutic interventions focusing on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction.
• Document patient responses to treatment and maintain accurate records for coordinated care.
• Manage your caseload and set your own schedule based on availability and patient needs.
• Use a simple EMR system for all notes, with tools to minimize documentation time.

Requirements

• Valid Arizona Physical Therapy professional license.
• CPR or BLS certification.
• Valid driver’s license and auto insurance.
• Physical and TB test (within the last year).
• Professional liability insurance and business license (if applicable).

Join our inaugural Arizona team and help establish our footprint in the culturally rich Greater Tucson area. This role offers geographic flexibility—focus on preferred neighborhoods or expand to adjacent areas.
