Home Health Physical Therapist (PT)

About the role

The Visiting Physical Therapist for our VNA Certified division evaluates, coordinates, and administers physical therapy services in patients' homes. Services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support.

Responsibilities

• Deliver physical therapy services according to prescribed physician orders.
• Follow policies and procedures, and adhere to all applicable patient care standards, licensure regulations, and state laws.
• Provide a written evaluation defining physical therapy needs, goals, progress, and expected outcomes in collaboration with the referring physician's Plan of Treatment.
• Participate in coordinating services, including discharge planning, with the health care team, physicians, parents/guardians, nurses, etc.
• Maintain effective communication among staff, healthcare team members, and families.
• Serve as a resource consultant for clinical staff.
• Supervise other personnel (Physical Therapy Assistant, Home Health Aide) as applicable.
• Complete OASIS assessments where permitted by state professional practice and regulation.

Requirements

• Certification in Physical Therapy in the state of application.
• Current and unrestricted license in the state of practice.
• Current CPR certification (with hands-on component) or ability to obtain one.
• Current TB test or ability to obtain one.
• Valid driver's license and reliable transportation.

• Experience delivering physical therapy services in home settings.
• Knowledge of state and federal regulations governing physical therapy.
• Ability to communicate effectively with patients, families, and healthcare providers.
• Strong organizational and time management skills.
