Home Health Physical Therapist PRN

About the Role

The Home Health Physical Therapist provides personalized in-home physical therapy to help patients regain strength, mobility, and independence. Reporting to the Clinical Manager, you will plan and administer treatment, assess patient strengths, develop rehabilitation programs, supervise PTAs and aides, and document patient progress.

Responsibilities

• Deliver prescribed physical therapy treatments in patients' homes to optimize physical function.
• Assess and document physical strengths to support treatment goals.
• Create and implement rehabilitation plans in collaboration with physicians and care teams.
• Provide clear care instructions to PTAs and aides, overseeing their performance and documentation.
• Maintain thorough records of interventions, observations, and patient progress.
• Report patient advancements to physicians, care teams, patients, and families, including evaluations and discharge summaries.

Requirements

• Valid and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience working with healthcare teams.

Qualifications

Preferred: At least one year of experience as a Physical Therapist.
