A Home Health Physical Therapist provides personalized physical therapy care in patients' homes to improve mobility, strength, and independence. The role involves developing and implementing treatment plans, assessing patient progress, documenting interventions, and coordinating with healthcare team members. This patient-facing position emphasizes personalized care, teamwork, and documentation to support patients' recovery and independence in a home setting.

Responsibilities

• Plan and administer treatments to optimize physical function
• Assess and document patient strengths and progress
• Develop rehabilitation programs aligned with physician plans
• Supervise PTAs and aides with clear instructions and oversight
• Report patient status and outcomes to the care team and families

Requirements

• Unrestricted Physical Therapy license
• CPR certification
• Prior experience collaborating with healthcare professionals
• One year of PT experience preferred
