Home Health Physical Therapist Assistant - Full Time

About the Role

The Physical Therapy Assistant provides skilled education, treatment, and interventions under a physician's plan of care in a home health setting. They collaborate with an interdisciplinary team, updating on patient progress to optimize well-being.

Responsibilities

  • Deliver direct physical therapy as directed by the physical therapist and physician.
  • Communicate regularly with patients and caregivers regarding the plan of care.
  • Administer treatments to relieve pain and restore or develop function.
  • Report patient observations and condition changes to supervisors or physicians.
  • Educate patients and caregivers on device use and rehabilitation techniques.
  • Coordinate with other service disciplines on patient care plans.
  • Plan and communicate discharge procedures and instructions.
  • Maintain documentation, including clinical notes and progress reports.
  • Ensure confidentiality and adhere to professional standards.
  • Participate in continuing education and comply with licensing requirements.

Requirements

  • Graduate from an accredited Physical Therapist Assistant program.
  • Current, unrestricted state license.
  • Valid CPR and driver’s license.
