About the Role
The Physical Therapy Assistant provides skilled education, treatment, and interventions under a physician's plan of care in a home health setting. They collaborate with an interdisciplinary team, updating on patient progress to optimize well-being.
Responsibilities
- Deliver direct physical therapy as directed by the physical therapist and physician.
- Communicate regularly with patients and caregivers regarding the plan of care.
- Administer treatments to relieve pain and restore or develop function.
- Report patient observations and condition changes to supervisors or physicians.
- Educate patients and caregivers on device use and rehabilitation techniques.
- Coordinate with other service disciplines on patient care plans.
- Plan and communicate discharge procedures and instructions.
- Maintain documentation, including clinical notes and progress reports.
- Ensure confidentiality and adhere to professional standards.
- Participate in continuing education and comply with licensing requirements.
Requirements
- Graduate from an accredited Physical Therapist Assistant program.
- Current, unrestricted state license.
- Valid CPR and driver’s license.
