Home Health Physical Therapist Assistant - Full Time

About the role

As a Physical Therapy Assistant, you will deliver skilled education, treatment, and interventions under the guidance of a physical therapist and physician, focusing on patient safety and well-being in a home health setting. You will collaborate with interdisciplinary team members, providing regular updates and documentation to ensure optimal care.

Responsibilities

• Administer direct physical therapy treatments according to the plan of care.
• Communicate progress and updates to patients, caregivers, and team members.
• Provide education on prosthetic, orthotic, and assistive device usage and rehabilitation techniques.
• Report patient observations and changes in condition to supervisors and physicians.
• Assist with discharge planning, instructions, and follow-up care.
• Maintain accurate records, documentation, and confidentiality in compliance with professional standards.
• Participate in in-service training and adhere to ethical guidelines.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current, unrestricted state license as a Physical Therapy Assistant.
• Valid CPR certification and driver’s license.
