Role: Physical Therapist Assistant supporting home and community-based care. Responsibilities Assist clients with therapeutic exercises, gait, balance, and functional mobility under supervision of a licensed Physical Therapist.Follow established care plans to deliver personalized care.Monitor client progress and document visits accurately.Communicate changes and updates to the care team to ensure coordinated support.
