Home Health Physical Therapist Assistant

Role: Physical Therapist Assistant supporting in-home rehabilitation services. Responsibilities Implement treatment plans under the supervision of a licensed Physical Therapist.Guide patients through therapeutic exercises and monitor their progress.Document outcomes in compliance with regulatory and company standards.Collaborate with caregivers and the clinical team to ensure effective care.Educate patients and families on safe mobility and home exercise programs.Assist clients in maintaining independence and quality of life at home. Qualifications & Skills Compassionate and patient-focused approach.Knowledge of rehabilitation techniques and therapeutic exercises.Strong communication and collaboration skills.Ability to follow treatment plans and document accurately.Experience working with older adults or individuals with chronic or disabling conditions is preferred.
