Home Health OT Occupational Therapist Part Time

Position Overview

Sought is an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with an interdisciplinary team to optimize patient well-being through evaluations, goal setting, and treatment planning.

Key Responsibilities

• Conduct initial and ongoing assessments of mobility, coordination, and function.
• Administer and interpret tests to establish functional goals and outcomes.
• Develop written evaluations and plans of care, documenting progress and modifying goals as needed.
• Provide occupational therapy services in accordance with physician orders.
• Cook discharge planning, including educating patients and caregivers on home exercises.
• Collaborate with healthcare team members, physicians, and families to ensure comprehensive care.
• Maintain documentation per policy, with minimal supervision, ensuring accuracy for billing purposes.
• Supervise personnel such as OT Assistants or Home Health Aides as applicable.
• Complete OASIS assessments where permitted by regulation.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Unrestricted state license as an Occupational Therapist.
• Valid CPR, driver’s license, and proof of automobile insurance.
