Home Health Occupational Therapist OT Part Time

About the Role

Join a dedicated home health team providing personalized occupational therapy services to diverse patients, focusing on dignity, outcomes, and continuity of care.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and functional ability.
• Develop and implement individualized plans of care with measurable goals and outcomes.
• Deliver occupational therapy services per physician orders, documenting progress and modifying treatment plans as needed.
• Coordinate discharge planning, including patient and caregiver education on home exercises.
• Collaborate with healthcare teams, including physicians and nurses, to ensure comprehensive care.
• Maintain accurate documentation to support billing and compliance, including OASIS assessments where permitted.
• Supervise other personnel such as OTA and HHA, if applicable.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist.
• Valid CPR certification.
• Preferred experience in home health therapy and OASIS documentation.
